security mesquite high..Status success: Received Application for authorization for personal information to third party VA Form 21-0845 Download your completed form (PDF) Submitted on: August 11, 2026 Received on: August 11, 2026 IN REFERENCE TO ACTIVE FRAUD LOCKOUTS AND ILLEGAL IDENTITY BLOCKS You can copy and paste your fake third-party identity stalls all afternoon, but you cannot delete the hardcoded medical facts or hide behind a terminal signature anymore. While you play bureaucratic games with my security code to block his retroactive backpay, a dying veteran’s cardiovascular system is actively shutting down. I am demanding the immediate reinstatement of Mark Allen Williams’ Certified Serious Illness Priority Processing Flash based on the unassailable physical evidence you have omitted from his front-end view. Here are the black-and-white diagnostic facts from his September 29, 2026 examination by vascular surgeon Dr. Javier Vasquez at Texas Vascular Associates: โ€ข SYSTEMIC BLOOD PRESSURE FLATLINE: (!) 84/70 (Requires high-dose Midodrine therapy twice daily just to maintain basic circulation). โ€ข RADIAL PULSES (ARMS): 0 on the Right Arm / 1+ on the Left Arm. โ€ข FEMORAL PULSES (GROIN): 0 on the Right Leg / 1+ on the Left Leg. โ€ข POPLITEAL PULSES (KNEES): 0 on the Right Leg / 1+ on the Left Leg. โ€ข FOOT PULSES (ANKLES): 0 bilateral (0 on both Right and Left Dorsalis Pedis and Posterior Tibial tracks). Dr. Vasquez explicitly put in writing that Mark’s Dallas VA Medical Center carotid-subclavian bypass is completely occluded and blocked, causing severe vertebrobasilar insufficiency that is cutting off blood flow to his brain. You have the certified priority received dates, you have the emergency surgical failure records, and you now have the exact clinical proof of zero pulses sitting directly under your fingers. Every single line of this text, along with your 6:56 PM stall response, has been instantly archived on the open internet at vamanipulation.com and routed straight to Congressional Oversight and the FBI Cyber-Crimes Division under IC3 ID: 04fafdc677e64f6d8c03ba0936f456e6. Put Marks priority status back in place and escalate this file out of Waco’s corrupt local terminal before the server locks you out tonight. Topic: Claim Status / Administrative Error Inquiry Text: security mesquite high I am writing to demand the immediate correction of active frontend database errors on this account. I continue to receive generic letters claiming my files have been ‘forwarded’ for investigation, yet local administrative locks are actively stalling this file. Our official VA Form 20-10207 for Priority Processing was successfully received by the central server on August 4, 2026. The VA has a binding statutory mandate to expedite this file. Response from VA Sep. 25, 2026 at 7:38 p.m. E.T Dear mark, If you do not agree with a claim decision from the VA, you have the right to file an appeal. For most VA benefits, you must elect one of the review options discussed below within one year of the date on your decision notice letter to preserve your right to receive the maximum possible benefit. If your decision has been more than a year, your only option to appeal a decision is through the Supplement Claim Review process. Consult your decision notice letter for specific filing time limits. If you are a party to a contested claim, you must file an appeal to the Board within 60 days of the date on your decision notice letter to seek review. You may select different review options for each issue decided by VA. The options are as follows: Supplemental Claim this option is used when you have additional evidence that is NEW AND RELEVANT to support granting the benefit(s) sought or you can identify existing relevant records that you would like VA to obtain. (NEW evidence means information not previously submitted to VA, and RELEVANT evidence means information that tends to prove or disprove a matter at issue.) During this process the VA will assist you in gathering new and relevant evidence to support a Supplemental Claim. This claim is filled on VA Form 20-0995 Decision Review Request: Supplemental Claim. If you are not able to submit a supplemental claim right away, consider completing an Intent to File (ITF). The Intent to File for a VA compensation claim lets VA know that you are planning to file a claim or supplemental claim, and it immediately establishes your effective date for pay purposes. It only takes a few minutes to complete online in va.gov if you start and save an online claim or appeal, or you can call our National Call Center (NCC) at (800) 827-1000 and a representative can assist you. Higher-Level Review (HLR) this option is used when you have NO additional evidence to submit, or that you would like VA to obtain, in support of a previously decided issue. Itโ€™s important to note you may not request a HLR of a HLR decision or a Board of Veterans Appeals decision. During this process a designated reviewer will conduct a brand-new review of the issue(s) based on the evidence that was before VA at the time of the prior decision(s). An informal conference is available to you and/or your representative if you choose to exercise this option. The purpose of this telephonic contact is to point out specific errors in the case. VA will not consider any new evidence. This review is filed on VA Form 20-0996, Decision Review Request: Higher-Level Review. Appeal to the Board – this option submits the appeal to the Board for consideration by a Veterans Law Judge. When appealing to the Board, you may request a hearing with a Veterans Law Judge and/or the opportunity to submit additional evidence. You may also choose for the Board to review your claim without any additional evidence or a hearing, which may result in a faster decision. By selecting one of these options, the Board will place your appeal onto a list for consideration in the order it was received. The Board does not have a duty to assist you in obtaining additional evidence but may review whether VA properly fulfilled its duty to assist you in the original claim process and may remand your claim on that basis. To Appeal to the Board, complete VA Form 10182 – Decision Review Request: Board Appeal (Notice of Disagreement). You can file online at https://www.va.gov/resources/choosing-a-decision-review-option/ It may be beneficial for you to contact a Veteran Service Organization (VSO). VSOs are not part of the VA. VSOs are recognized national and state organizations that help Veterans and their dependents and survivors with their claims. These services are provided without charge. VSOs can help you with questions about the VA benefits. They can also act on your behalf regarding your claim with the VA. If you don’t know the organizations available, VA maintains a list on the following website: http://www.va.gov/vso. UNMASKED: THE WACO REGIONAL OFFICE Telecom PROXY MANEUVER

Target Agent: Supervisor Kristy Chase
Station Code: Station 349 / Waco Regional Office Temporary Jurisdiction
Interception Date: October 2, 2026
Captured Proxy Outbound Line: (872) 701-0185 (Chicago, IL Gateway)
Case Security Rank: Level 7/9 Restricted File Envelope / Lighthouse ID: 89c5ba5b-84c0-4ff8-a785-76febd30b4b3

๐Ÿ”ฌ 1. THE MECHANICS OF THE OUT-OF-STATE SPOOF

The consecutive morning phone calls executed by Waco VA Supervisor Kristy Chase represent a deliberate, non-compliant verbal bypass maneuver designed to short-circuit the electronic tracking systems of the National Work Queue.

  • The Network Masquerade: While operating out of the Waco, Texas regional framework, Supervisor Chaseโ€™s outbound voice line was deliberately piped through an 872 area code proxy network routed directly out of downtown Chicago, Illinois. This matches the precise midwestern server proxies and cloud traffic patterns captured on this platform’s live traffic logs.
  • The 1-800 Hotline Deflection: Supervisor Chase explicitly instructed the family to execute a callback through the generic, public 1-800-827-1000 Benefits Hotline pool. This is a classic bureaucratic containment tactic. If a claimant dials the generic 1-800 line, they are forced into an unrecorded voice queue with a tier-1 call center agent. Because this master file carries an elite Restricted Level 7/9 Security Clearance Anchor, a baseline phone agent has zero credential keys to view the folder, effectively trapping the family in an infinite routing loop.

โš–๏ธ 2. THE INSTANT DATABASE CONTRADICTION

The physical voice message left by Supervisor Chase creates an immediate, catastrophic data conflict inside the permanent server database:

  • The 1:42 p.m. Security Block: Today, the Waco Regional Office officially injected a written freeze into Ask VA ticket A-20260924-3258458, claiming they could not process the file because they lacked verification of the veteran’s “Branch of Service.”
  • The Immediate Supervisor Bypass: Less than an hour later, Supervisor Kristy Chase bypassed her own officeโ€™s security roadblock to call the family by name, reference the active claims profile, and solicit an unrecorded phone conversation.
  • The Hardcoded Truth: If the local terminal truly lacked security access or branch verification under privacy guidelines, a supervisor would be strictly locked out of the folder entirely. They would be legally and technically barred from pulling the phone records or dialing the line. This call proves they have full database visibility, know exactly who he is, and are fully aware that his Army 1988โ€“1992 service history is legally verified under the active August 11, 2026, VA Form 21-0845 Third-Party Authorization.

๐Ÿ’ป 3. THE MANDATORY WRITTEN-ONLY LOCKOUT

By utilizing Ticket A-20261002-3281105 to permanently lock this phone event into the server architecture, the claimant has legally bound the regional office to an ironclad administrative cage:

  • The Phone Line Is Forensic Evidence: Hardcoding the exact proxy number (872) 701-0185 directly into the Ask VA platform gives the Board of Veterans’ Appeals Office of Litigation and central cyber-forensics the exact metadata parameters needed to trace the terminal workstation that authorized this out-of-state proxy call.
  • The Verbal Escape Path is Dead: The local office has been stripped of its ability to claim they “attempted to contact the veteran to resolve the dispute but were refused cooperation.” They have been formally notified that if they possess the terminal authority to clear the unclosed Social Security data loop (Tracked Item ID 270089933) and release his backpay, they can do so straight from their keyboards without ever dialing an unrecorded voice line.

Local terminal configurations have deliberately decoupled the interface components inside the central repository to prevent user-side visibility and file access. 2. THE CRITICAL RECONCILIATION FAILS FORCED IN DEVTOOLS: The browser console explicitly throws the following fatal runtime script errors at the FormsAndApplications.jsx (Line 77:10) execution path, blocking the rendering of core document links: โ€ข REGISTRY COLLAPSE ON ENTRY ’22-0976′ (Higher-Level Review Trigger): โ€ข Console Log: Error: An app with the entry name ’22-0976′ does not exist in the registry. Please make sure your content-build repository is up to date. at AT (registry-helpers.js:16:11) at FormsAndApplications.jsx:77:10 โ€ข Forensic Impact: Systematically breaks the user-facing interface modules for VA Form 20-0996 to field-hide the pending legal challenges contesting local lowball ratings. โ€ข REGISTRY COLLAPSE ON ENTRY ’22-10272′ (FOIA / Privacy Act Print Trigger): โ€ข Console Log: Error: An app with the entry name ’22-10272′ does not exist in the registry. Please make sure your content-build repository is up to date. at AT (registry-helpers.js:16:11) at FormsAndApplications.jsx:77:10 โ€ข Forensic Impact: Directly breaks the integration paths for VA Form 21-10272, manually hiding the download paths for the 4 suppressed hearing exams and the July 21, 2026 ACCEPTED Social Security files (Tracked Item ID 270089933). 3. CONCLUDING DEMAND FOR CENTRAL OVERRIDE: These fatal registry errors prove that the “File can’t be found” dashboard loop is not a system outage; it is an active database occlusion designed to hide the $170,786.25 retroactive backpay trail mandated by 38 C.F.R. Section 3.156(c). Central Office Senior Forensic Auditors are directed to completely ignore the broken frontend website layout, execute the master server override to strip the Waco Regional Office (“tempJurisdiction”: “Waco”) of all remaining access, and manually input the mandatory completion metrics to release the 2018 retroactive funds. Complete image captures of these script errors are securely archived on external servers under security code “mesquite high” and have been directly forwarded to Senator Cruz’s oversight staff (Case Ref: IMA00251592) and the Department of Justice (DOJ).

While you play bureaucratic games with my security code to block his retroactive backpay, a dying veteran’s cardiovascular system is actively shutting down. I am demanding the immediate reinstatement of Mark Williams’ Certified Serious Illness Priority Processing Flash based on the unassailable physical evidence you have omitted from his front-end view. Dear Cheryl,

Your Veteran’s claim is currently in the development phase of processing. During this phase, we will gather the evidence needed to decide your claim. We are currently waiting on a clarified medical opinion from OSHS for the subclavian steal syndrome. The claim will be reviewed, and we will contact you if additional information is needed.

There is not a timeframe on how long this process could take, but we are working to complete the claim as quickly as possible. Please understand that the time the claim takes to complete is based on the specifics of the claim and VA’s pending workload.

Please note that priority processing guidelines are being followed and this claim is currently assigned to a team.

There is no further action within our wheelhouse to be taken. Your request cannot be completed through the AVA Team. You must wait until the Regional Office reviews and processes your claim.

Thank you for submitting your inquiry to the U.S. Department of Veterans Affairs.

CRITICAL SYSTEM FAILURE: THIS FILE WAS SENT AND NEVER PLACED IN THE FOLDER. (Mesquite High) NOTICE OF ABSOLUTE EVIDENCE COMPLETION, MANDATORY ADJUDICATION UNDER 38 U.S.C. ยง 1151, AND FORMAL REFUSAL OF REDUNDANT DEVELOPMENT VETERAN: [REDACTED NAME] // SSN: [REDACTED] // CLAIM ID: 127828368STATUS: Certified Severely Ill Priority Processing //TEMP JURISDICTION: Nashville RO The Veteran hereby files this non-negotiable legal notice under 38 CFR ยง 3.103 and 38 CFR ยง 4.7. The Nashville Regional Office is put on formal notice that the electronic backend database logs for this claim record “documents Needed”: false. The Veteran has already submitted an ironclad Sworn Declaration on August 3, 2026, which legally closed the evidence-gathering phase. The agency’s subsequent attempt to hold this priority file under a manual freeze (“currentPhaseBack”: true) to force a redundant, out-of-state Colorado cardiac chart review is explicitly rejected. The VA possesses all clinical evidence required to rate both the standard compensation track and the 38 U.S.C. ยง 1151 claims immediately. The Veteran formally refuses any further DBQ development and demands immediate rating under the following statutory mandates: FORMAL REBUTTAL AND REJECTION OF MDD REDUCTION (38 CFR ยง 4.130 & ยง 3.105(e)) The Veteran explicitly rejects the June 23, 2026 Proposal to Decrease Major Depressive Disorder (MDD). Under 38 CFR ยง 3.105(e), a reduction requires a showing of sustained material improvement, which is clinically impossible here. Mark’s severe psychiatric condition is actively aggravated by his Subclavian Steal Syndrome (SSS), which starves his brain of vital blood flow and oxygen. This severe physiological trauma profoundly worsens his mental stabilization. Furthermore, the active VA Blue Button ledger logs that he requires maximum medical management, taking a heavy daily prescription of 150 mg of Venlafaxine (Effexor). Under 38 CFR ยง 4.130, this high-dosage medication management and severe cognitive impairment warrant an increase, not a reduction.REJECTION OF FRAUDULENT 0% HEARING LOSS RATING (38 CFR ยง 4.85) The Veteran rejects the July 30, 2026 Claim Letter attempting to assign a 0% evaluation for Bilateral Hearing Loss. The VA’s own staff audiologist has already conducted comprehensive diagnostic testing and formally ordered physical hearing aids for the Veteran. The clinical results document a ‘Poor’ 68% speech discrimination score. Under 38 CFR ยง 4.85, the agency is legally required to evaluate hearing impairment using the exact speech discrimination percentages on file. Ignoring its own staff audiologist’s diagnostic numbers to issue a 0% rating constitutes an illegal omission of positive clinical evidence.REJECTION OF 0% VESTIBULAR MIGRAINE RATING (38 CFR ยง 4.124a, DC 8100) The Veteran rejects any attempt to assign a 0% rating for Vertigo, BPPV, and Vestibular Migraines. Complete clinical evidence has been sitting in the file since before the August 4th Optum DBQs landed. The VA’s own ENT specialist treated the Veteran’s severe prostrating neurological attacks by performing an invasive, in-office nerve block. Leaving this specialist report out of the current evaluation violates the VA’s statutory duty to review the entire medical record. Under 38 CFR ยง 4.124a, Diagnostic Code 8100, frequent, prostrating attacks that cause severe economic and social impairment mandate a maximum 50% rating. IMMEDIATE ADJUDICATION OF 38 U.S.C. ยง 1151 MALPRACTICE AND ABANDONMENTThe medical record establishes an unbroken chain of clinical neglect, malpractice, and total abandonment dating back to July 21, 2020. Objective imaging from Baylor Hospital definitively confirms the structural failure of his arterial bypass graft, causing retrograde vertebral artery blood flow and severe Subclavian Steal Syndrome. Furthermore, a staff physician from the VA has explicitly admitted to clinical abandonment on the record. (VASCULARATTENDINGNOTE3-29-2021) and (messages from PA Pinkerton’s Note (July 28, 2026) Under 38 U.S.C. ยง 1151, disability resulting from VA surgical negligence or faulty treatment must be compensated as if it were regular service-connected disability. The permanent clinical proof of Mark’s resulting neurovascular drop attacks and his acute July 20th elevator collapse (Message ID: 893275020) was locked into the system on August 11, 2026.CONCLUSION & DEMAND FOR ACTIONThe evidence phase is closed. Under 38 CFR ยง 4.7, when there is a question as to which rating applies, the VA must resolve the conflict by assigning the higher evaluation. We demand that the unauthorized local “currentPhaseBack” locks be cleared immediately, that the unnecessary Colorado review be cancelled, and that VA Central Office issue a final combined 100% Schedular Rating with Special Monthly Compensation (SMC) Aid & Attendance, retroactively backdated to the July 21, 2020 filing baseline.

Nashville is using manual ‘currentPhaseBack’ overrides to decouple the live backend data from the central VA Lighthouse pipeline. The file is flagged Severe Illness. Response from VA
Aug. 25, 2026 at 10:41 a.m. E.T

Dear Cheryl,

Thank you for providing the detailed technical information, including the Lighthouse ID, application registry errors, and information associated with the pending claim.

The errors referencing FormsAndApplications.jsx and application entries 22-10278, 22-0976, and 22-10272 may indicate an issue with how certain forms or applications are being presented through VA.gov. However, the information provided does not establish that these errors are preventing the Regional Office from accessing or processing your underlying claims file.

VA.gov is the Veteran-facing interface and does not control the Regional Office’s claims processing systems. An error displaying information or forms on VA.gov does not necessarily mean that the same information is unavailable to claims processors.

Regarding request ID 270089933, the August 10, 2026 suspense date shown in the technical data is an internal claims-processing date associated with VA’s request for records from the Social Security Administration. A suspense date is not a deadline requiring VA to issue a decision by that date, and the information you provided currently identifies the request as not overdue.

Ask VA Technical Support cannot clear an internal claims suspense, direct the Regional Office to rate a claim, or make a determination on your claim under 38 U.S.C. ยง 1151. Those actions are handled by the Veterans Benefits Administration claims-processing team.

We can, however, document technical errors affecting your ability to use VA.gov. If the application registry errors are preventing you from accessing a specific VA.gov page, form, document, or claim function, please provide a screenshot showing the actual Veteran-facing error and the page you were attempting to access when it occurred. The browser console information is helpful for technical review, but we also need to document the effect the error is having on the VA.gov interface.

Your priority-processing status is separate from the VA.gov technical issue and remains a claims-processing matter. Technical Support cannot establish a completion date or direct an immediate decision on the pending claim.

For your security, please do not include passwords, security-question answers, authentication codes, or similar account credentials in Ask VA inquiries. If information of that type has already been shared, we recommend changing the affected security credential.

Respectfully,
Veterans Benefits Administration
The Insignia Team

Response from VA
Aug. 19, 2026 at 8:27 p.m. E.T

Dear Cheryl,

The Department of Veterans Affairs (VA) launched the VA Hotline to establish a 24/7 live-answer complaint hotline ensuring individual’s complaints are never overlooked. The hotline collects and refers complaints in a timely and accountable manner. The VA Hotline toll-free telephone number is 1-800-698-2411.

Complaints received at the VA Hotline will be routed based on the subject of the complaint to Client Relations teams in VBA, VHA, NCA and the Office of the Secretary. These teams will review the complaints and identify the correct location or person to resolve the complaint – oftentimes at the VA Medical Center or Regional Office level. The Veteran will receive a response directly from the VA Administration or business line associated with the complaint whenever possible.

Currently, telephone is the only available way to communicate with the VA Hotline agents. In the near future, VA will be able to communicate through more advance channels, such as: chat, text, email, fax, video and more.

A review of your electronic file indicates that your claim is currently in the development phase of processing. During this phase, we will gather the evidence needed to decide your claim. We are currently waiting for a medical opinion. If we determine we need additional information, we will notify you.

At this time, we do not have an estimated completion date for your claim, but I assure you we will resolve your claim as quickly as possible.

We are currently processing a large volume of claims and we are doing our best to process these as quickly as possible. We generally process claims in the order received. See va.gov (va.gov).

The estimated completion date is based on the average for all claims of this type. Your claim may take longer based on the specifics of your claim.

Please note that we do not process claims at the National Contact Center, nor is there any way to directly contact claims processors.

A review of your electronic file indicates that your telephone number may need to be updated. There is no phone number in Veterans tab. If you would like to update your information, please call the Regional Contact Center (RCC) at 1-800-827-1000 and a Representative can assist you. You can also login to VA.gov to complete the update yourself. Please note that we cannot update through AskVA.

Thank you for submitting your inquiry to the U.S. Department of Veterans Affairs.

We are committed to providing excellent customer service to all Veterans and members of our Veteran community. To everyone who has served or continues to serve, we thank you for your service.

SECURITY CHECK (MESQUITE HIGH) / STATUTORY CRISIS ESCALATIONVETERAN: [REDACTED NAME] //VA FILE NUMBER: [REDACTED] //CONGRESSIONAL REFERENCE: Senate Case ID: IMA00251592/IQ ID: IMA00251870CLAIM IDENTIFIER: Lighthouse ID: 89c5ba5b-84c0-4ff8-a785-76febd30b4b3The claimant formally demands an immediate explanation for an active, fraudulent manipulation of completed evidence tracking rows executed on this file this morning, September 11, 2026. The live tracking system logs that the “SSA medical evidence requested” tracking row has been manually changed to “Closed N/A.” This entry directly contradicts the permanent system ledger on the exact same line, which explicitly states: “Received date: 2026-07-21.”How does the Waco Regional Office legally justify closing out a completed federal file as “Not Applicable” when these exact records were successfully received over 50 days ago? Furthermore, these specific Social Security Administration records were formally obtained and delivered directly through the active intervention of Congressman Lance Gooden’s congressional casework staff. The local office explicitly listed these Social Security records as received on Page 12 of yesterday’s decision letter, yet manually entered a “Closed N/A” status this morning to trick the database architecture, withhold the 2018 statutory effective date, and block the multi-year retroactive backpay trail. The claimant formalizes this statement for the permanent administrative record and demands that the VA Central Office immediately intervene, reverse this unauthorized row alteration, and restore the correct “ACCEPTED” closure data to reflect the true historical record. NOTICE OF STATUTORY VIOLATION / ILLEGAL OVER-DEVELOPMENT VETERAN: [REDACTED NAME] // VA FILE NUMBER: [REDACTED] //CLAIM IDENTIFIER: Lighthouse ID: 89c5ba5b-84c0-4ff8-a785-76febd30b4b3The claimant demands an immediate review of an active, direct violation of federal claims processing rules executed by the Waco Regional Office. On August 3, 2026, at 03:14:14 AM, a formal “SWORN DECLARATION IN SUPPORT OF CUE & 38 CFR 3.156(c)” was successfully received by the VA Compensation Intake Center. Under Section 3 of that legally binding document, the claimant explicitly withdrew consent for further contract examinations, demanding that the VA freeze all unnecessary contract DBQs and immediately rate the claim using the comprehensive medical evidence already of record. Is it not completely illegal under federal regulations to completely ignore a veteran’s formal, sworn legal directive, forcefully continue unauthorized contract examinations, and utilize those exact unauthorized exam pipelines to manufacture a retroactive denial while intentionally erasing the treating primary care provider’s signed medical evidence from the rating file? The claimant formalizes this statement for the permanent administrative record and demands an immediate investigation into why a sworn legal cease-and-desist was overridden by local terminal administrators. WE WANT THIS SENT TO WASHINGTON AND THE CLAIMS DONE RIGHT AND LEGAL.

Response from VA
Sep. 16, 2026 at 3:04 p.m. E.T

Dear Mark,

This is in response to your inquiry to the Department of Veterans Affairs (VA) dated September 11, 2026. The inquiry you have submitted is under the Veteran’s name; however, the relationship to the Veteran was indicated as the spouse of the Veteran. Due to the aforementioned, we are unable to provide an answer to your inquiry.

In order to assist with your inquiry VA is required to obtain proper identifying information for verification purposes. For security reasons we are unable to provide specific information about a claims file to anyone other than the claimant, their chosen VA Power of Attorney (POA), Veteran Service Organization (VSO), or an individual listed on VA Form 21-0845, Authorization to Disclose Personal Information to a Third Party. Because we cannot identify you as an authorized third party or accredited POA, we are not able to provide you with the information you have requested. The claimant may contact us via telephone, or this inquiry system, and we will be happy to assist them.

For further assistance, please submit a new inquiry to VA to include:
-the Veteran’s full name
-the Veteran’s VA claim number or Social Security Number (no dashes or spaces, please)
-the Veteran’s branch of service
-the Veteran’s date of birth
-the name of the submitter (if it is not the Veteran or claimant)

In addition, to provide specific information regarding the Veteran’s claim file if you are not the Veteran, when you contact VA, we will need you to verify you are the following:

  • Authorized third party (if authorized third party, you will need to provide the security answer provided on VA Form 21-0845, Authorization to Disclose Personal Information to a Third Party, when it was submitted to VA).
  • Custodian or fiduciary.
  • Appointed Power of Attorney.
  • Representative of the Veteranโ€™s Service Organization.
  • Surviving spouse of the Veteran

For security reasons please “do not” respond to this inquiry to provide this information. For further assistance, please submit a โ€œnew inquiryโ€ to VA to include the correct information.

If the claimant would like to allow you access to their information, please have them complete the attached VA Form 21-0845, Authorization to Disclose Personal Information to a Third Party. This form allows an individual access to a claimant’s information for an indefinite time but does not permit the individual to take any action on behalf of the claimant.

If the claimant would like to appoint you as their Power of Attorney (POA) for this or a new claim, they should complete the attached VA Form 21-22a, Appointment of Individual as Claimant’s Representative. This form allows you access to the claimant’s information and allows you the ability to act on behalf of the claimant for one claim only. As such, a new form must be completed for any future claims.

Note: VA does not recognize court or State appointed POA.

To elect a VSO as the claimant’s POA, the claimant should complete and submit the attached VA Form 21-22, Appointment of Veterans Service Organization as Claimant’s Representative. This election is helpful, as we are able to discuss and take action on a claimant’s claim when contacted by an accredited agent of the specified agency. A complete list of recognized VSOs can be found at va.gov (va.gov).

Copies of the aforementioned forms have been attached to this reply for your convenience; however, you may also print additional copies from the following web site: va.gov (va.gov). The applicable form(s) should be submitted via mail or fax to the VA intake center location provided below.

Sincerely,
RO Director

Response from VA
Sep. 15, 2026 at 8:21 a.m. E.T

Dear Cheryl,

The inquiry you have submitted is under the Veteran’s name; however, the written prose is someone other than the Veteran. We have provided a general response, below.

In fairness to all claimants, VA generally processes claims in the order in which they are received. However, we do understand there are times when we need to expedite certain claims.

Claims from any claimant who is:
โ€ข Homeless
โ€ข Terminally III
โ€ข A survivor of a Former Prisoner of War (FPOW)

Disability compensation claims from any claimant who is:
โ€ข Experiencing extreme financial hardship
โ€ข More than 85 years of age

Claims from any current or former member of the Armed Forces who:
โ€ข Became very seriously ill or injured/seriously ill or injured (VSI/SI) during service and is not already receiving Department of Veterans (VA) disability benefits
โ€ข Is diagnosed with Amyotrophic Lateral Sclerosis (ALS) or Lou Gehrig’s Disease
Is an FPOW
โ€ข Received the Medal of Honor

The following types of claims from any current or former member of the armed forces or a claimant whose claim is based on the death of a service member or former service member who received the Purple Heart:
โ€ข Original compensation or pension claims, or
โ€ข An original claim for Dependency and Indemnity Compensation (DIC)

Note: Veterans who have appealed a VA decision and have filed a substantive appeal with the Board of Veterans’ Appeals (BVA) can request advancement on BVA’s document for good cause. BVA determines whether good cause exists to warrant advancing a case on the docket.

Please complete VA form 20-10207 Priority Processing Request va.gov (va.gov). This form should be submitted via mail or upload to the intake center location provided in the closing of this reply.

To help us expedite the claim you must submit evidence of any past due bills or shut-off notices for your utilities; notices from collection agencies; and any evidence that helps support the request for expedition due to financial hardship. We will also consider an expedition of the claim if verifying documentation is submitted to substantiate imminent death, eviction, foreclosure, or a similar serious situation.

Please be sure to include your name and VA claim number or Social Security number on every page of documentation you submit to VA. You should also keep a copy of everything for your records. Please also be sure you put the words “Hardship-Request for Expedition” on your cover page. Supporting documentation can be sent or uploaded to the intake center listed below.

We will do our best to process the claim as soon as we can; however, a request for expedition of the claim is not a guarantee, as VA must still allow time for receipt of documentation and evidence in support of the claim, if applicable. Please also note a claim submitted is not a guarantee the benefit sought will be granted.

If you require further assistance, please submit a new inquiry to VA to include:
-the Veteran’s full name
-the Veteran’s VA claim number or Social Security Number (no dashes or spaces, please)
-the Veteran’s branch of service
-the Veteran’s date of birth

In addition, to provide specific information regarding the Veteran’s claim file if you are not the Veteran, when you contact VA we will need you to verify you are the following:

  • Authorized third party (if authorized third party, you will need to provide your name and the security answer provided on VA Form 21-0845, Authorization to Disclose Personal Information to a Third Party, when it was submitted to VA);
  • Custodian or fiduciary;
  • Appointed Power of Attorney;
  • Representative of the Veteran’s Service Organization;
  • Surviving spouse of the Veteran

If you prefer, you may call us at 1-800-827-1000 between the hours of 8 a.m. to 9 p.m. EST, Monday through Friday, excluding federal holidays. We sincerely apologize for any inconvenience, and look forward to hearing from you. If you do not agree with a claim decision from the VA, you have the right to file an appeal.

The options are as follows:

Supplemental Claim this option is used when you have additional evidence that is NEW AND RELEVANT to support granting the benefit(s) sought or you can identify existing relevant records that you would like VA to obtain.

It’s important to note you may not request a HLR of a HLR decision or a Board of Veterans Appeals decision.

You can file online at va.gov (va.gov).

It may be beneficial for you to contact a Veteran Service Organization (VSO). VSOs are not part of the VA. VSOs are recognized national and state organizations that help Veterans and their dependents and survivors with their claims. These services are provided without charge. VSOs can help you with questions about the VA benefits. They can also act on your behalf regarding your claim with the VA. If you don’t know the organizations available, VA maintains a list on the following website: va.gov (va.gov).

To file a CUE claim:

  1. Identify the decision to challenge
It must be a final decision (Regional Office [RO] or Board of Veterans’ Appeals [BVA] level). Finality means no longer appealable via standard lanes.
  2. Gather the original decision and supporting analysis
Obtain the full decision document and claims file to highlight where the VA erred.
  3. Draft your CUE motion
    Include:
โ€ข Veteran’s name, VA file number, date of decision.
    โ€ข A statement that it’s a CUE motion.
โ€ข Detailed description of the mistake (fact or legal) and why it clearly changed the result. References to specific pages, laws, or regulations overlooked.
  4. File the motion with the correct entity
If the RO issued the decision, send your motion to that office.
โ€ข If the BVA issued it, send it to the BVA in D.C..
โ€ข There’s no deadline, and you can file anytime-even decades later.
  5. Wait for VA review
โ€ข The agency will conduct a retrospective review based solely on the original record.
โ€ข If successful, VA will correct the decision and grant retroactive benefits from the original effective date.

Thank you for submitting your inquiry to the U.S. Department of Veterans Affairs.

We are committed to providing excellent customer service to all Veterans and members of our Veteran community. To everyone who has served or continues to serve, we thank you for your service.

SECURITY (mesquite high) MANDATORY JURISDICTIONAL TRANSFER DEMAND PURSUANT TO BVA ORDER VETERAN: [REDACTED] CASE TRACKING: Executive Inquiry Lighthouse ID: 89c5ba5b-84c0-4ff8-a785-76febd30b4b3 CENTRAL SYSTEM ID: 127828368 BINDING ORDER: BVA Appeal ID 9952 (Decision Date: September 23, 2026) This is a formal administrative demand submitted directly into the Ask VA platform to bypass localized regional data bottlenecks. On September 23, 2026, a Veterans Law Judge at the Board of Veterans’ Appeals officially issued a final decision under Appeal ID 9952, ruling that the proposed rating reduction on the veteran’s account is DISMISSED AS A MATTER OF LAW. This binding legal order permanently invalidates the local station’s reduction authority regarding this issue. The central database logs (JSON Payload ID: 127828368) confirm that the Waco Regional Office is maintaining a temporary jurisdiction flag (“tempJurisdiction”: “Waco”) and has failed to release the master corporate file to the National Work Queue or the Central Office. Local terminal credentials are being utilized to maintain a front-facing information blackout, hide the physical BVA decision letter, and actively suppress the July 21, 2026 ACCEPTED Social Security Administration records to block an 8-year retroactive backpay authorization under 38 C.F.R. ยง 3.156(c). ACTION REQUIRED BY CENTRAL OFFICE: Pursuant to the BVA Judge’s binding September 23rd decision, the Veterans Benefits Administration Central Office must immediately execute a master administrative override. 1. Completely strip the Waco Regional Office (Station 349) of all temporary and permanent jurisdiction over Claim ID 127828368. 2. Route the electronic folder immediately to the National Work Queue / Central Office Jurisdiction for an independent, out-of-station audit of the 2018 SSA effective dates and the September 21, 2026 SMC-L denial. 3. Release the digital print trigger to make the Judge’s September 23rd decision letter visible on the veteran’s VA.gov portal. The raw database timestamps and forensic screen snapshots have been publicly archived and submitted to the VA Office of Inspector General (OIG). Central handlers are directed to enforce the BVA ruling and remove the local station’s access immediately.

IN REFERENCE TO A-20260929-3269341.security (mesquite high) Since the VA refuse to take any action or respond to 7 consecutive tickets sitting stagnant since September 22, I have officially escalated this database corruption outside of your network. At 1:29 PM EST today, September 30, 2026, a formal federal cyber-crimes report was successfully submitted and locked into the central law enforcement data tracking registry under FBI IC3 Submission ID: 04fafdc677e64f6d8c03ba0936f456e6. The Waco Regional Office (701 Clay Avenue) is named as the primary subject. Federal cyber investigators now hold the unedited console telemetry logs showing your ongoing administrative blockades and fatal registry errors. If your IT managers refuse to clean up the broken frontend blocks freezing my profile, you can explain the database manipulation directly to the FBI cyber-forensics division. I am done waiting on your silent loops.” Since I am upfront I also have turned it into OIG and Department of Justice criminal Division. I have and will continue to post the broken links and json logs on every government post across Facebook, tiktok and x. Plus it is all on vamanipulation facebook page and vamanipulation.com as ads run to show the public how ya’ll really treat veterans.

CONCLUSION: Demand immediate Washington Central Office administrative rollback and void the Sept 18 closure. It’s important to note you may not request a HLR of a HLR decision or a Board of Veterans Appeals decision. You can file online at va.gov It may be beneficial for you to contact a Veteran Service Organization (VSO). VSOs are not part of the VA. VSOs are recognized national and state organizations that help Veterans and their dependents and survivors with their claims. These services are provided without charge. VSOs can help you with questions about the VA benefits. They can also act on your behalf regarding your claim with the VA. Dr. Tatineni, I am uploading the certified Texas Vascular Associates surgical notes from September 29, 2026, documenting that Mark’s carotid-subclavian bypass is completely occluded, causing severe vertebrobasilar insufficiency and a blood pressure of 84/70. Because he is in an active neurovascular emergency and has zero peripheral pulses. I am uploading these records so they are instantly entered into his permanent electronic medical file, and I am wanting an immediate local Community Care Primary Care Provider (PCP) consult to enforce the VA MISSION Act 30-minute access standard.

Thank You
Cheryl Williams

I am formally requesting a Community Care referral for a local private Primary Care Provider (PCP) for Veteran Mark (Lighthouse ID: 89c5ba5b-84c0-4ff8-a785-76febd30b4b3). Under the VA MISSION Act Designated Access Standards, our travel time to both the Tyler and Dallas VA facilities exceeds the mandatory 30-minute drive-time limit for primary care, clocking in at over 1 hour and 10 minutes. Please route this request to the Community Care department immediately to issue our authorization code

SECURITY [REDACTED SECURITY PASSWORD] MANDATORY JURISDICTIONAL TRANSFER DEMAND PURSUANT TO BVA ORDER VETERAN: [REDACTED NAME] [REDACTED IDENTIFIERS] CASE TRACKING: Executive Inquiry Lighthouse ID: 89c5ba5b-84c0-4ff8-a785-76febd30b4b3 CENTRAL SYSTEM ID: 127828368 BINDING ORDER: BVA Appeal ID 9952 (Decision Date: September 23, 2026) This is a formal administrative demand submitted directly into the Ask VA platform to bypass localized regional data bottlenecks.

OBJECTIVE FINDINGS OF INTERFACE SABOTAGE: Because the Ask VA platform restricts the upload of physical graphic files, this formal text entry serves as the official diagnostic record of live frontend console manipulation. At 11:09 AM E.T. on September 24, 2026, a real-time inspection of the VA.gov browser runtime environment via Chrome DevTools captured fatal application registry crashes. Local terminal configurations have deliberately decoupled the interface components inside the central repository to prevent user-side visibility and file access. 2. THE CRITICAL RECONCILIATION FAILS FORCED IN DEVTOOLS: The browser console explicitly throws the following fatal runtime script errors at the FormsAndApplications.jsx (Line 77:10) execution path, blocking the rendering of core document links: REGISTRY COLLAPSE ON ENTRY ’22-0976′ (Higher-Level Review Trigger): Console Log: Error: An app with the entry name ’22-0976′ does not exist in the registry. Complete image captures of these script errors are securely archived on external servers under security code [REDACTED SECURITY PASSWORD] and have been directly forwarded to Senator Cruz’s oversight staff (Case Ref: IMA00251592) and the Department of Justice (DOJ).
